Purpose
This is an invitation for you to participate in a research project under the direction of Nikki Prosch, SDSU Extension Health and Physical Activity Field Specialist, in the Department of Health and Nutritional Sciences at South Dakota State University.  The project is entitled, “The Impact of Physical Activity on Stress and Mood in Farmers.”  The purpose of the project is to assess various factors including physical activity, perceived stress, and mood that impact the health and wellness of farmers and ranchers.  You must engage in farming and ranching at least part-time, and you must be 18 years old or older to participate in the project.

Procedures
If you consent to participate, you will be asked to complete a survey, which will take approximately 10-15 minutes of your time. The survey will be administered either via paper and pencil or online.  Participation in this project is voluntary. You have the right to withdraw at any time without penalty, and you have the right to skip any questions you do not wish to answer. Please DO NOT put your name on this survey.  The survey is anonymous and will be kept confidential. Questions about protocols and procedures may be directed to the study personnel administering the survey or to the Project Director.
Benefits
While there are no direct benefits to you for participation, indirect benefits include acquiring a better understanding of your own mental and physical health perceptions and behaviors. Additionally, by participating in the study, you are aiding in the development of a greater body of knowledge regarding the experience of farming and ranching with designing prevention efforts to help alleviate stress in farmers and ranchers.

Risks
While there are no known risks related to participation in the study, some questions are sensitive in nature and may be upsetting to read. If you experience emotional distress, the project director will provide a list of mental health resources.  

Compensation
To thank you for your time, participants who complete the survey will receive a $10 Visa Gift Card.  If you are completing this survey online, please click on the link at the bottom of the survey.  A second survey will appear where you will provide your name and mailing address.  The two surveys cannot be connected, so your responses cannot be tied directly to your name and mailing address.  A $10 Visa or MasterCard Gift Card will be mailed to you.  If you are completing the survey in person at an agricultural fair, a $10 Visa or MasterCard Gift Card will be handed to you upon completion.

Confidentiality
Your responses are strictly confidential. All responses on the survey are anonymous. When the data and analysis are presented, you will not be linked to the data by your name, title or any other identifying item. Your responses will be anonymous to ensure that they cannot be linked to you personally. By completing the survey, you are indicating that you, as a research participant, are 18 years old or older, have read the above, have had any questions answered, and agree to participate in the research project.  Your consent is implied by the return of the completed survey. You may request a copy of this form for your information.
